How It Works
From your first call to active support, we make every step clear and stress-free.
The process begins with your regional center service coordinator. Reach out to them to start a conversation about additional support options for your family.
Your service coordinator will explain what Coordinated Family Support is, how it works, and whether it may be a good fit based on your individual's needs and goals.
Once referred, our team meets with the individual and family to conduct a thorough assessment — understanding daily life, strengths, challenges, and what support looks like for you.
We develop a person-centered care plan with clear goals and begin providing consistent, hands-on support — adjusting as your family's needs evolve over time.
